New: Reflection on Risk Form for Self-Employed LPNs

The CLPNM Practice Direction on Self-Employed Practice requires that self-employed LPNs conduct a risk analysis before establishing a self-employed practice to inform their service planning, procurement of equipment, harm prevention strategies, emergency management, and the development of policies, procedures and protocols. This risk assessment and management plan must be reviewed regularly, and updated at least annually.

The CLPNM has developed a new risk assessment tool to be used by self-employed LPNs for this purpose.

If you are a self-employed LPN, recent amendments to the Practice Direction on Self-Employed Practice, and the Practice Direction on the Continuing Competence Program, require that you complete this form and maintain a copy in your professional portfolio.

The completed form does not need to be submitted to the CLPNM, unless:

  • The CLPNM specifically requests it. The form may be requested by the CLPNM at any time, including during the Continuing Competence Program (CCP) Audit.
  • You are a self-employed LPN who has been selected for the Quality Assurance Audit (Category 2 of the Continuing Competence Program Audit) and you do not have another regulated health professional who can provide feedback on your practice. If this is the case, you may submit your completed Reflection on Risk Form as an alternative to having another regulated health professional provide an evaluation of your practice using the CLPNM’s Practice Feedback Form.
